EMPLOYMENT
In order to work in the State of Hawaii you will need to complete this google form: KHIS GOOGLE FORM
Certificate of Age Work Permit (16 & 17 years of Age)
You will need your social security number and an email (not your Kahuku email) address. After the work permit is submitted online, the Department of Labor will email your work permit to you as an attachment.
Print out the work permit and take it to the employer.

Minor's Certificate of Employment Form (14 & 15 years of Age)
For students ages 14 and 15, a Minor's Certificate of Employment form can be obtained at the College & Career Center Q2 or online at:

Download the form, complete it, and submit it with the following documents: current State ID, Social Security Card, Birth Certificate, and/or Passport.
