Extended Absence Approval Process

A parent/guardian must complete the Extended Absence Request Form. LINK: Extended Absence Request Form

After submitting, confirm the AdobeSign verification email by clicking the link. This step is required for the request to be sent to the Vice Principals, and you will receive a PDF copy of the submission.

The Vice Principal will meet with the student and provide a printed copy of the request.

The student must take the form to each teacher to review required work and obtain teacher signatures.

The completed form must be returned to the Vice Principal by the due date listed on the form, at least 2 days before the absence.

The Vice Principal will review the request and notify teachers if the absence is approved.
Note: The form must be completed using a parent/guardian email account. Submissions from a student KahukuHS account cannot be processed.
By submitting an Extended Absence Request, you agree to the following:

If an absence exceeds 10 days, only the first 10 days will be excused.

Extended absences will not be approved during finals week or during seniors’ last week of classes.

Students are responsible for all work assigned during the absence, including work previously assigned (such as History Day, Science Fair, projects, or summative assessments), which may still be required during the absence.

Completed work must be submitted upon return or according to a timeline agreed upon with the teacher prior to the absence.

Students and families are responsible for communicating with teachers about missed instruction, assignments, and deadlines.

Teachers are not responsible for replicating missed instruction and may assign alternate work if needed.

Students may be required to attend Action Period or other scheduled times to complete missed quizzes, tests, labs, or receive additional help.
